How to obtain a marriage record in Florida?

  • Download an application or request form from the Records Bureau’s online portal.
  • Fill out the form with all the significant details required. Include the complete names of the groom and bride, and the date and county of marriage. If the state filing number is known, please indicate that on the form as well.
  • As the one requesting the record, you have to include your full name, address, and your contact number. Please include your relationship to the subjects and your reason for obtaining such record.
  • Prepare $5 for the processing fee. Such payment should be through check or money order made payable to the Department of Health.
  • Once everything is ready, mail your order along with the corresponding fee to the Department of Health.
